No. The haggard woman, I believe, is one of those aspects that you realize made sense after you've seen the movie and start thinking back on it. She puts her fingers to her lips in a "shhh" gesture, apparently because she knows a big secret, which the audience (and Leo's character) don't until the end.

Keeping in mind that (spoilers) there is no Rachel Solando, I did think the old woman at the beginning did look a little like Solando 1 (the one who turns out to be a nurse). It was a different actress, though I bet the effect was intentional, just to give a sort of creepy vibe (I have no idea what else it might have meant).
