Susan Foreman: On 22nd century Earth.
Victoria Waterfield: Living with Frank and Maggie Harris in 1968.
Vicki Pallister: In Ancient Greece, 1200 B.C.
Leela: On Gallifrey.
Rose Tyler: On Earth in a parallel universe.with the Meta-Crisis tenth Doctor.
Amelia Pond and Rory Williams: In New York City, 1938 (?)

I think Steven Taylor might fit into this category. He definitely ended up on a different planet.

Let's see:
Katarina - out of an airlock so in space I guess
Sara Kingdom - aged to death on the planet Kembel
Steven Taylor - left in the Savages on an unknown planet
Dodo - Sent to the country in the War Machines but essentially home
Ben and Polly - Back on the same day and the same city they left from, not bad.
Jaime and Zoe - Both returned to their own place
Liz - never travelled
Jo - home
Sarah Jane Smith - probably not even in South Croydon
Harry - home
K9 I - On Gallifrey with Leela
K9 II - With Romana in E-Space
K9 III - With Sarah Jane Smith
As only K9 I really had a home other than the TARDIS, probably just one not home.
Romana - E-Space
Tegan - 3 years on but home
Nyssa - No home to return to. Stays on Terminus
Kamelion - destroyed on Sarn
Turlough - On the planet Sarn but heading home to Trion
Peri - marries King Yrcanos (perhaps, maybe, there are many outcomes)
Melanie Bush - Joins Sabalom Glitz to keep him on the straight and narrow
Ace - Still travelling
